Why does the embryo not implant? – Understanding implantation failure

Dr. med. Wibke WilkeningNovember 20255 min

You have already had one or more IVF/ICSI attempts, but the embryos have not implanted? That is frustrating – but there are options for a work-up.

What is implantation failure?

When no pregnancy occurs after at least three transfers with good-quality embryos, this is referred to as recurrent implantation failure (RIF).

Possible causes

  • Endometrial factors: the endometrium is not optimally prepared.
  • Immunological factors: including an elevated concentration of plasma cells in the endometrium.
  • Coagulation disorders: can impair blood flow to the endometrium.
  • Embryonic factors: chromosomal abnormalities.

What we can do

  • ERA test: determines the optimal window for the transfer.
  • Plasma cell diagnostics: detects chronic endometritis.
  • Coagulation diagnostics: thrombophilia screening.
  • Hysteroscopy: rules out polyps, septa or fibroids.

Our experience

Many patients become successfully pregnant after targeted work-up and adjusted therapy. Do not give up.
